Job Title: Social WorkerLocation: ShropshireRate: Up to £34 per hourOur client in Shropshire is looking for an experienced Social Worker to join their Intermediate Care Services Team.
What will your responsibilities be?In this role, you will conduct thorough social care assessments to identify the needs of patients preparing for discharge. You will develop and implement effective care plans tailored to individual needs. Collaborating with medical, nursing, therapy, and discharge teams, you will ensure smooth and timely discharge plans for patients. You will manage a caseload, providing timely and effective support for each patient. Working with partner organizations and colleagues, you will provide effective outcomes that meet the assessed needs of residents. You will carry out Mental Capacity Assessments and make Best Interest decisions as required. Additionally, you will provide prompt support to meet immediate risks and needs in crisis situations and identify, investigate, and manage potential safeguarding issues to protect adults from abuse and neglect.

Qualifications and ExperienceTo be successful in this role, you will need a recognized professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW (Certificate of Qualification in Social Work). You must be registered with Social Work England, the regulatory body for social workers in England. Significant experience in delivering social work services to vulnerable adults, including those with complex needs, is essential. You should have a thorough understanding of relevant legislation, such as the Care Act 2014 and the Mental Capacity Act 2005. Strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team, are crucial. Additionally, a full UK driving licence and access to a car are often required for client visits.
