Support Coordinator - Creative Support Ltd : Job Details

Join the Knowsley High Intensity User Service team as a Support Coordinator and make a difference to individuals experiencing mental or physical health crises.

As part of our team, you must be willing to embrace all aspects of the role including, but not limited to, providing support with personal care and domestic tasks, Your role will include, developing the independent living skills and community inclusion of service users, identifying and accessing activities with service user to promote valued and meaningful lifestyles, providing respectful personal care, emotional and practical support to service usersc, collaborating with service users, families, and the multi-disciplinary team to maximise service user outcomes and supervising and supporting staff to best meet service user needs and preferences

Working in partnership with various agencies and community groups, you’ll help improve health and wellbeing for our service users, ensuring they receive inclusive and non-discriminatory care that meets their diverse needs.

You will be required to work in a flexible manner, on a roster basis, which will include evenings, weekends and sleep-ins. Previous practitioner experience in working with vulnerable adults is essential; however what we need most from you is gumption and a can-do attitude.
