A South London Council is excited to offer a rewarding opportunity for a dedicated, experienced, and qualified social worker to join their Adult and Learning Disability Services team.

The role is part of our dynamic and supportive team, where you'll collaborate closely with Children's Services, Education, and Health to ensure that young individuals receive the right support as they prepare for adulthood.
The focus of this role is primarily on supporting the assessment and support planning for high-cost placements and care packages for young people approaching adulthood who have learning disabilities.
You will be responsible for conducting Care Act 2014 compliant eligibility assessments for young people and their carers. This requires a timely identification of social care needs and the creation of person-centered, strengths-based outcomes within support plans. Additionally, you will carry out comprehensive Continuing Healthcare (CHC) checklists, working collaboratively with NHS partners to secure CHC funding for eligible young people.
An essential aspect of your role will be monitoring, reviewing, and maintaining up-to-date support plans. These plans must evolve to meet the changing needs of young individuals and ensure that the support provided is cost-effective while advocating for the young person's rights, choices, and access to services. As a qualified social worker, you will be expected to safeguard and promote the welfare of young people, strictly adhering to relevant legislation and policies. This includes assessing levels of risk and need in complex situations and conducting safeguarding investigations when necessary. In some cases, your work may involve proceedings in the Court of Protection, where you'll be responsible for delivering protective and/or supportive services for individuals at risk.
You will also manage a complex and varied caseload, making sound professional judgments with appropriate supervision and guidance. This role requires a proactive approach to managing cases in line with policy, procedures, and legislation. Collaboration is key in this position, and you will work closely with multidisciplinary teams, including children's services, education, healthcare professionals, and community partners, to ensure a holistic approach in meeting the needs of the young person.
