Position: Home Support WorkerLocation: StockportTeam: REaCH (End of Life Services)Pay Rate: £12 to £12.92 per hour

About the Role:

REaCH has exciting opportunities to join a fast-paced, hard-working team dedicated to delivering the highest quality person-centred support to those in receipt of its services. We are looking to recruit additional Home Support Workers to work within our End-of-Life services to meet the increasing demands for end-of-life and palliative care.

Key Responsibilities:

• Support to maintain and develop relationships.
• Encourage individuals to be independent and make informed choices.
• Administer or assist with medications.
• Support with challenging and complex behaviour.
• Maintain accurate records.
• Provide emotional support to family members.
• Uphold the core values of REaCH and Stockport Council’s Adult Social Care services to ensure service users' final wishes to remain at home are met.

About You:

Are you caring and a good communicator?Are you able to listen and talk to our clients?Do you have a desire to support and help people in the community?Do you have a full UK driver’s license and access to a reliable vehicle insured for work purposes?

Essential Skills and Qualifications:

• Good understanding of safeguarding and care plans.
• Excellent communication skills.
• Ability to drive with a full UK license and business insurance.
