The Role:

As our Transition Coach, you will work with potential students who need additional support to successfully make the move to College.

The Transition Coach will work in conjunction with the SENDCo, teaching staff, applicants and parents to provide tailored support from initial visits through to College induction.

Responsibilities as our Transition Coach:

  • Providing support and guidance through the pre-entry processes to potential students and parents/carers, to advise on support available at College and help plan for next steps. Develop a good understanding of the curriculum and support programmes The College can offer.
  • Assisting in the consultation process liaising with local authorities and external professionals to review individual applicants support needs and plan steps to College.
  • Supporting students transition to College through agreeing and implementing a plan of activities with all professionals working with the student.
  • Working with staff at feeder schools to identify the students most likely to need additional support with managing the transition to college and agree support plans and visits,
  • Coordinating transition support for an agreed caseload of students.  Monitoring effectiveness whilst providing continued support to the students, and staff, until such time as the students are settled on their programmes.
  • Motivating, encouraging and supporting the student to socially engage and integrate within our college community. This may include supporting students in activities as part of the student’s course or through cross-college events.
  • Participating in student induction, college orientation and obtaining accurate course timetables prior to start of term to ensure support is in place for Day 1 plans.

As our Transition Coach, you’llideally have:

  • Level 2 (or equivalent) English and Maths
  • Education/training to level 2 (e.g. GCSE, NVQ2)
  • Previous experience working with people with learning difficulties, disabilities and/or challenging behaviours
  • Experience working in a busy environment to manage a caseload, working to key deadlines
  • Awareness or training supporting your knowledge of Special Educational Needs and Disabilities
  • The ability to plan and organise events, multitasking between projects
